Affinity Visiting Nurses, a member of Ministry Home Care, began providing hospice services to our community 25 years ago with the visionary work of Dr. Charles "Chris" Wallace, as he co-founded the Appleton Community Hospice with Mary Runge of the VNA. Over the years, the hospice program has changed management (and therefore names) several times, but Dr. Wallace's vision continues to flourish, helping individuals with end-stage terminal illnesses live their final days with peace and dignity.
Affinity Visiting Nurses has been providing home health services to the Fox Valley community for over two decades as well, working with patients and their families to promote and restore health and independence, while minimizing the effects of the illness or disability, in the comfort of their home.
We provide complete home health and hospice services to individuals in portions of the following Wisconsin counties: Brown, Calumet, Fond du Lac, Green Lake, Manitowoc, Marquette, Outagamie, Shawano, Sheboygan, Waupaca, Waushara, and Winnebago.
Our expert interdisciplinary staff will come to the patient's home, nursing home, or CBRF to provide services as prescribed by the patient's personal physician. Services provided include skilled nursing care by registered nurses, physical therapy, occupational therapy, speech/language pathology, medical social work support, and assistance by home health aides. In addition, hospice care includes supportive services from our spiritual counselors, specially-trained volunteers, and bereavement counselors. |
